Amendment of the contract / statute

• KRS number of the company whose contract/statute is to be changed,

• the latest consolidated text of the Company's articles of association/statutes,

• description of changes in the articles of association/statutes of the Company.

• whether the Assembly has been formally convened.

• in the case of a general partnership / partnership / limited partnership - data
  Partners, i.e.: (names, surname, parents' names, marital status, political system
  spouse's property number, ID card or passport number and expiry date, PESEL number,
  address of residence), and in the event that a person is to be a party to the contract
  legal (or other entity) – full company/name with data.

Dissolution of the company

• KRS number of the company whose contract/statute is to be terminated,

• the latest consolidated text of the Company's articles of association/statutes,

• data of the liquidators, unless the dissolution of the Company will take place without conducting
  liquidation proceedings.

• whether the Assembly has been formally convened.
